(Sink is included in Kitchen Equipment Item 3 above).

6. THE Vinyl Wall Covering called for on A-7, Room Finish Schedule shall be an

Allowance Item for materials only = $15 Linear Yard (54” roll width) for Type II VWC

min 20oz. weight.

7. DELETE ALL PREVIOUS REFERENCES TO WAGE SCALE RATES, AND USE THE

ATTACHED “GENERAL DECISION: TN20080014 06/05/2009 TN14” document dated

June 05, 2009.

8. DRAWINGS, Sheet A-1, Lower Level-A ADD 4” perforated schedule 40 CPVC

foundation drain pipe, filter cloth, Foundation Waterproofing, and protection board

below grade on the East, North and West faces of the foundation. Connect rigid

pipe to 4” flexible perforated drain pipe and run to daylight on South end of site.

Install waterproofing to East, North, and West walls – W.R. Grace Bituthene 3000

and Hydroduct 225 Drainage composite board in manufacturer’s approved

installation or approved equal. Refer to attached Lower Level Plan – A-1a.

9. All bidders shall submit the attached BID FORM with TOTAL LUMP SUM BID,

ADD ALTERNATES 1 & 2, and acknowledgement of receipt of all ADDENDUM. See

the attached Bid Form.

10. ADD ALTERNATE No. 2

See attached 11x17 format sheet A-1a for items identified as to include the

following:

a. Relocate Entry Doors and concrete stoops and Change one 3’-0”x7’-0” to 6’-0”

x 7’-0”.

b. Add one 8’-0” x 4’-0” Hollow Metal Frame Window and Bullet Proof Glazing.

c. Lower Footing elevations for areas indicated by 12” for future computer

wired access floor (NIC).
